Physical Therapist – Per Diem

Position Summary
Avir at the French Home is seeking a compassionate and skilled Physical Therapist (PT) to join our team on a per diem basis. The Physical Therapist will provide evaluation and treatment services designed to help residents improve mobility, strength, balance, functional independence, and overall quality of life.
Essential Responsibilities
Evaluate residents to identify physical limitations, mobility concerns, and functional needs.
Develop individualized treatment plans based on each resident's condition and goals.
Provide therapeutic exercises, mobility training, balance activities, gait training, and other appropriate interventions.
Monitor resident progress and modify treatment plans as needed.
Educate residents, families, and caregivers on exercises, mobility techniques, and safety.
Collaborate with nursing staff, physicians, occupational therapists, speech therapists, and other members of the care team.
Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements.
Promote resident safety and independence during all therapy activities.
Participate in care planning and communicate changes in resident status to the appropriate team members.
Follow professional, ethical, and facility standards of practice.
Qualifications
Current Physical Therapist license in the State of Texas.
Graduate of an accredited Physical Therapy program.
Current CPR certification, as required.
Strong clinical, communication, organizational, and interpersonal skills.
Experience working with older adults or in a skilled nursing/rehabilitation setting is preferred.
Ability to work independently while collaborating effectively with an interdisciplinary team.
