About the Role
We are seeking a highly skilled and experienced Admissions Director to join our team at Vivo Healthcare Harbourwood. As the primary liaison between hospitals, families, physicians, case managers, and internal departments, you will be responsible for managing the referral, admission, and intake process for residents entering our skilled nursing facility. Your ultimate goal will be to ensure smooth transitions of care and timely admissions while promoting occupancy growth and compliance with facility policies and regulatory requirements.
Essential Duties and Responsibilities
Receive, review, and process referrals from hospitals, physicians, case managers, and community partners
Coordinate all aspects of resident admissions, including bed availability, payer verification, and clinical acceptance
Communicate with prospective residents and family members regarding services, admissions requirements, and facility expectations
Conduct facility tours for potential residents and families
Verify insurance coverage, Medicare, Medicaid, managed care, and private pay eligibility
Collaborate with nursing, therapy, dietary, social services, and other departments to prepare for new admissions
Ensure all required admission documentation is completed accurately and timely
Obtain and maintain physician orders, medical records, and transfer documentation prior to admission
Coordinate transportation and admission logistics when needed
Maintain census reports and admission tracking logs
Participate in marketing and community outreach activities to build referral relationships
Follow HIPAA guidelines and maintain confidentiality of resident information
Ensure compliance with federal, state, and facility admission regulations
Assist with discharge planning and readmission coordination when applicable
Participate in meetings related to census, occupancy, and referral development
Qualifications
High school diploma or GED required; Associate’s or Bachelor’s degree preferred
Experience in healthcare admissions, case management, customer service, or marketing preferred
Skilled nursing facility or long-term care experience strongly preferred
Knowledge of Medicare, Medicaid, managed care, and insurance verification
Strong communication and interpersonal skills
Ability to work in a fast-paced environment and manage multiple priorities
Strong organizational and documentation skills
Proficient in Microsoft Office and electronic medical record systems
Physical Requirements
Ability to sit, stand, walk, and move throughout the facility for extended periods
Ability to lift up to 20 pounds occasionally
Ability to use office equipment, including computer, phone, copier, and fax
