Nassau Rehabilitation and Nursing Center is dedicated to providing high-quality, resident-centered care in a supportive and collaborative environment. We are seeking a compassionate and skilled Occupational Therapist to join our interdisciplinary rehabilitation team.

Job Responsibilities

  • Assess patients’ physical, cognitive, and emotional abilities to identify areas for improvement

  • Develop and implement individualized treatment plans tailored to patient needs

  • Provide therapeutic activities and interventions to improve daily living and functional skills

  • Educate patients and families on coping strategies, adaptive techniques, and assistive devices

  • Collaborate with physicians, nurses, and other rehabilitation professionals to ensure comprehensive care

  • Monitor patient progress and modify treatment plans as needed

  • Maintain accurate documentation of evaluations, treatment plans, and progress notes

  • Advocate for patient needs across care settings

  • Assist patients with transitions back to daily activities

  • Conduct home or workplace assessments and recommend modifications when appropriate

  • Stay current with advancements and best practices in occupational therapy

Candidate Requirements

  • Bachelor’s or Master’s degree in Occupational Therapy from an accredited program

  • Current New York State Occupational Therapist license

  • Certification from the National Board for Certification in Occupational Therapy (NBCOT)

  • Minimum of 2 years of experience as an Occupational Therapist

  • Nursing home or skilled nursing experience preferred

  • Strong knowledge of human anatomy, physiology, and psychology

  • Proficiency with electronic health records (EHR), including Net Health and Sigma (preferred)

  • Excellent communication, problem-solving, and critical-thinking skills

  • Ability to work independently and collaboratively within a multidisciplinary team
