Creekside Care Center of Gahanna is looking for a dedicated and energetic Occupational Therapist (OT) to join our in-house rehabilitation team. We currently have opportunities for both Full-Time and PRN OTs.

What You'll Do

As an Occupational Therapist, you'll play an important role in helping our residents maintain and regain the skills needed for greater independence and quality of life.

Responsibilities include:

• Evaluate residents and develop individualized occupational therapy plans of care
• Provide skilled OT services in accordance with physician orders and established treatment plans
• Help residents improve independence with activities of daily living, mobility, safety, and functional performance
• Collaborate with PT, SLP, nursing, physicians, and other members of the interdisciplinary team
• Educate residents, families, and caregivers regarding treatment plans and recommendations
• Complete timely and accurate documentation using Net Health
• Participate in care planning and discharge planning
• Maintain compliance with facility, state, federal, and professional standards

What We're Looking For

• Graduate of an accredited Occupational Therapy program
• Current Ohio Occupational Therapist license in good standing
• Strong communication and teamwork skills
• Compassionate, resident-centered approach to care
• SNF/LTC experience is a plus, but we welcome OTs who are excited to grow within the setting
