To assist in achieving this mission, NAC is currently seeking an LPN: Pediatric Nurse/Care Coordinator to join its Comprehensive Healthcare Center (Medical).Title of Position: LPN: Pediatric Nurse/Care CoordinatorPosition Type: Full-timeSalary Range: $62,000- $67,000Location: 825 7th Avenue, New York, NY, 10019Department/Program: Comprehensive Healthcare Center (Medical)Responsibilities:In the communityMake home visits to children in the Prevention and Foster Care Programs, according to program guidelinesAccompany children and families to selected primary care and pediatric and surgical subspecialty visits, as neededIn our clinicTriage responsibilities include obtaining and documenting vital signs, collecting specimens, performing vision and hearing screens and administering immunizations for children served in the clinics, as ordered by MD/PNPCare coordination activitiesAssess, under RN supervision, the medical needs of each child through reviewing medical diagnoses, medical history, and inpatient and outpatient recordsProvide education for families on how to optimally care for the children in their home settings, particularly around the children's special health care needs, with reinforcement as neededCollaborate within the medical/nursing department and across departments with social workers, education specialists, recreation staff, mental health, and other professionals to best support meeting the child's health care needsMaintain and update plans of care as needed, assuring optimal quality of careParticipate actively in multidisciplinary Clinical Care Conferences and Family Team Conferences convened at NAC and selected interagency case conferences held at NAC and in the communityFacilitate access to medical, specialty and dental care in the community, seek medical records for all visits attendedOther duties:Ensure accurate and timely documentation of all patient care and community-based activities.Assist in administrative dues, when neededAssist in quality improvement activities involving services provided to children in assigned caseload, including maintaining up-to-date medical record documentationRequired Education and Experience:Completion of accredited licensed practical nursing program, completion of licensing examMinimum 1 yr of nursing experienceRequired Skills:Bilingual (English/Spanish) a plusComputer literacy requiredExperience with using electronic health records requiredFor information on New Alternatives for Children, Inc., visit our website at www.nackidscan.orgNew Alternatives for Children, Inc. is an equal opportunity employer, and all qualified applicants will receive consideration for employment without regard to race, color, religion, age, sex, national origin, disability status, genetics, veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.New Alternatives for Children, Inc. was founded on the principle of inclusion.
