Position Overview

Absaroka Senior Living Center is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our nursing team. This position is ideal for a nurse who takes pride in providing exceptional resident care while contributing to a collaborative and professional clinical environment.

As an LPN, you will provide direct nursing care, administer medications and treatments, monitor resident conditions, supervise and support nursing assistants, and communicate changes in condition to the appropriate members of the healthcare team.

You will work closely with Registered Nurses, CNAs, physicians, providers, and interdisciplinary team members to ensure residents receive safe, timely, individualized, and person-centered care.

Key Responsibilities
  • Provide direct nursing care in accordance with individualized care plans, physician/provider orders, facility policies, and established clinical standards

  • Perform ongoing observation and assessment of resident conditions within the LPN scope of practice and promptly communicate significant changes to the appropriate licensed nurse or healthcare provider

  • Prepare and administer prescribed medications, treatments, and therapeutic interventions safely and accurately within authorized scope of practice

  • Monitor residents for medication effectiveness, adverse reactions, changes in condition, and other clinical concerns

  • Maintain accurate, complete, and timely nursing documentation within the electronic medical record (EMR)

  • Implement physician/provider orders and ensure appropriate follow-up and communication

  • Collaborate with physicians, Registered Nurses, CNAs, therapy, dietary, social services, and other interdisciplinary team members to coordinate resident care

  • Supervise, delegate, direct, and support CNAs and other nursing personnel consistent with applicable scope-of-practice requirements

  • Conduct resident rounds to monitor care delivery, resident safety, comfort, and changes in condition

  • Communicate professionally and compassionately with residents and families regarding care needs and concerns within appropriate scope

  • Respond promptly to changes in condition, urgent clinical needs, and emergency situations in accordance with established protocols

  • Participate in admissions, transfers, discharges, and other care transitions as assigned

  • Support infection prevention and control practices, including hand hygiene, PPE, isolation precautions, and other established protocols

  • Participate in quality assurance, performance improvement, resident safety, and clinical compliance initiatives

  • Protect resident rights, dignity, privacy, preferences, and confidentiality

  • Immediately report suspected abuse, neglect, exploitation, mistreatment, injuries of unknown origin, or other resident-safety concerns in accordance with facility procedures and applicable requirements

  • Maintain compliance with applicable federal and Wyoming requirements, professional nursing standards, and facility policies

Qualifications
  • Graduate of an accredited practical nursing program

  • Current, active Licensed Practical Nurse (LPN) license in good standing in the State of Wyoming

  • Current CPR or Basic Life Support (BLS) certification, as required by facility policy

  • Long-term care, skilled nursing, rehabilitation, or geriatric nursing experience preferred

  • Strong clinical observation, critical-thinking, and problem-solving abilities

  • Ability to recognize and appropriately respond to changes in resident condition

  • Ability to effectively supervise, delegate, and provide direction to nursing assistants and support personnel

  • Strong medication administration and documentation skills

  • Excellent written and verbal communication skills

  • Ability to collaborate effectively with residents, families, providers, and interdisciplinary team members

  • Ability to organize and prioritize multiple responsibilities in a fast-paced clinical environment
