Position Summary
The Physical Therapist will provide skilled home health rehabilitation services designed to improve mobility, strength, balance, endurance, pain management, and overall functional independence for patients in their home environment.
Responsibilities
- Evaluate and treat patients with physical limitations, mobility impairments, pain, and functional deficits in the home setting
- Develop and implement individualized treatment plans in accordance with physician orders and established plans of care
- Provide skilled therapy interventions to improve strength, balance, gait, endurance, safety, and overall mobility
- Assess home environments and recommend modifications or adaptive equipment as appropriate
- Monitor patient response to treatment and document progress toward functional goals
- Communicate patient progress, concerns, and recommendations with physicians and interdisciplinary care team members
- Educate patients, family members, and caregivers regarding exercises, mobility, fall prevention, safety, and home programs
- Maintain accurate, timely, and compliant documentation according to company policy and state/federal regulations
- Participate in care coordination and interdisciplinary meetings as required
Qualifications
- Active and valid Texas Physical Therapist license in good standing
- Graduate of an accredited Physical Therapy program
- Home health experience preferred
- Skilled nursing facility (SNF) or geriatric experience preferred
- Knowledge of electronic documentation systems preferred
- Strong communication, organization, and interpersonal skills
- Ability to work independently and manage patient schedules effectively
