LTC Facility in Queens seeking a PT Dietitian. Qualifications- BS in Food & Nutrition or RD/ADA Eligible. Must have at least 3 years of Clinical Dietitian experience in LTC. Duties include- Sigma Electronic assessments, care plans, MDS's and progress notes; Gerimenu; visit and counsel Residents; make recommendations for diets and or supplements; knowledge of therapeutic diets and Tube Feedings; attend care plan meetings; communicate with kitchen supervisors and staff on Resident needs; Meal Observation; and cover Chief Clinical Dietitian duties when needed
