Responsibilities:
Conduct assessments and develop individualized nutrition plans for residents, taking into account their unique needs, preferences, and dietary requirements
Collaborate with the interdisciplinary care team to develop and implement care plans that promote optimal health outcomes
Provide nutrition education and counseling to residents, families, and caregivers
Monitor and assess the effectiveness of nutrition programs and make adjustments as needed
Develop and implement innovative nutrition programs, ensuring compliance with regulatory requirements and company standards
Maintain accurate and detailed records of resident nutrition plans and progress

Work Environment:
As a PART_TIME employee, you will work approximately 8 hours per week
