Job Summary:
In Home Rehab Group, a leading provider of outpatient physical therapy and occupational therapy services, is seeking a highly skilled and compassionate Occupational Therapist to join our team. As part of our In-Home Treatment program, you will have the opportunity to provide personalized occupational therapy services to patients in the convenience of their own homes. Responsibilities:
Provide occupational therapy services to patients in their homes, focusing on functional skills and daily living activities
Develop and implement individualized treatment plans, in collaboration with patients, families, and healthcare providers
Conduct thorough assessments and evaluations to identify patients' needs and goals
Utilize a variety of treatment techniques and strategies to promote patient progress and independence
Maintain accurate and timely documentation of patient progress and treatment plans
Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
Participate in team meetings and professional development activities to stay current with best practices and industry developments

Requirements:
Bachelor's or Master's degree in Occupational Therapy
Current New York State licensure as an Occupational Therapist
Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
Excellent communication and interpersonal skills
Ability to work effectively independently and as part of a team
