Responsibilities:

• Provide skilled occupational therapy services under the direction and supervision of a licensed Occupational Therapist

• Carry out individualized treatment plans designed to improve patients' independence, safety, and quality of life

• Assist with therapeutic exercises, activities of daily living (ADLs), adaptive techniques, and use of assistive equipment

• Monitor patient progress and communicate changes to the supervising Occupational Therapist

• Educate patients and caregivers on home safety, exercises, and techniques to maximize independence

• Complete accurate, timely clinical documentation

• Participate in case conferences, team meetings, and quality improvement initiatives

• Collaborate with physicians, nurses, therapists, and other healthcare professionals to provide coordinated patient care

• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes

Qualifications:

• Graduate of an accredited Occupational Therapy Assistant (OTA) program

• Current Certified Occupational Therapy Assistant (COTA) license in the state of practice

• Current NBCOT certification, if applicable

• Current CPR certification

• Minimum of two years of COTA experience preferred

• Home Health experience is a plus but not required

• Valid driver's license, reliable transportation, and auto insurance
