Physical Therapist – Home Health (Contract)Location: Oregon 97216
Pay: $50 – $53/hr
Schedule:
Position Summary & Responsibilities:
Evaluate patients and develop individualized home treatment plans
Provide therapy interventions to restore/improve physical, social, and mental function
Establish and communicate care and discharge plans with patients/families and team
Supervise therapy assistants as needed
Complete accurate and timely clinical documentation
Ensure patient safety and adhere to quality standards
Perform other duties as assigned
Requirements:
Doctor of Physical Therapy (DPT) or equivalent
Active Physical Therapist license in Oregon
BLS certification (AHA or equivalent)
Valid driver’s license and at least 21 years of age
