Responsibilities:
Provide direct patient care under the supervision of a registered nurse, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor and record patient conditions, reporting any changes or concerns to the nursing staff and healthcare provider
Communicate effectively with patients, families, and colleagues to ensure seamless transitions and continuity of care
Participate in quality improvement initiatives and maintain accurate records and documentation

Requirements:
Current LPN license in the state of Michigan
Basic Life Support (BLS) certification
Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
Excellent communication and interpersonal skills
Strong attention to detail and accuracy in record-keeping and documentation
