Job Summary
We are seeking a full-time Speech Language Pathologist/CF for a Skilled Nursing Facility. The SLP/CF will work closely with the adult population with complex communication needs. Great opportunity to join a collaborative rehab team in a supportive SNF environment. Mentored experience transitioning from clinical fellow to independent professional.
Job Responsibilities
Conduct comprehensive assessments and provide evidence-based treatment of speech, language, cognitive and swallowing abilities.
Develop individualized treatment plans tailored to each patient’s needs.
Collaborate with families, educators, and healthcare professionals to support patient progress.
Complete accurate and timely documentation including daily notes, progress reports, evaluations, and discharge summaries.
Stay current with advancements in speech therapy practices and medical terminology.
Ensure compliance with facility protocols and state/federal regulations.
Experience & Qualifications:
Master’s degree in Speech Language Pathology
Valid NY SLP License. CFs are welcome to apply!
Strong communication and organizational skills contributing to a collaborative environment
Knowledge of dysphagia and cognitive rehabilitation preferred
Experience in a skilled nursing facility preferred
Experience with Net Health/Rehab Optima is a plus
