Job Summary:

Astoria Healthcare Center is seeking a compassionate and organized Concierge to provide exceptional customer service to our residents and their families. As a key member of our team, you will be responsible for ensuring a seamless transition for new admissions, providing personalized services, and maintaining ongoing communication with families. Job Responsibilities:

NEW ADMISSIONS:

  • Check in to see status for upcoming and previous day admissions

  • Inspect room before admission (bed made and working, clean, bedside table and drawers, phone, remotes)

  • Welcome new admission

  • Give new admission a welcome packet

  • Tell resident about Facility, Activities, and show how to use telephone, TV, and electric bed

  • Discuss daily schedule at Facility and medication and treatment delivery

  • Set up IDT meeting and therapy schedule

  • Complete inventory list

  • Provide name ID band and complete section of Base Line Care Plan

  • Assist with obtaining signatures for admission packet

  • Bring Social Worker to room and introduce to resident (discuss discharge plan)

  • Introduce resident to care givers – CNA, LVN, RN

MENU - DIETARY:

  • Order resident and family admission meal – using resident diet order for resident and family choice for request – serve meals in room

  • Go over menu with them

SHORT TERM RESIDENTS:

  • Keep track of short-term residents and follow up with each of them every day

  • Maintain ongoing communication with family via phone, email – send picture in therapy, send picture in activities etc. & follow-up with resident and family after 1 week and as necessary

DISCHARGES:

  • Visit residents being discharged – It is critical that every resident that is discharged feels they had a great experience at the Facility

  • Complete a Customer Service Survey before every discharge

  • Grievance process and follow up to ensure resolution

  • Complete inventory list

  • Assist with transportation set ups and follow ups for resident appointments

  • Report to Administrator

GENERAL:

  • Answer phones

  • Answer call lights

  • Keep facility clean

  • Keep track of all items of note (big or tiny) in a binder and share with Admin and DON

ADDITIONAL DUTIES:

  • Assist with admission packets

  • Backup accounts payable

  • Backup payroll

  • Manage facility social media accounts

QUALIFICATIONS:

  • A CNA Certification is preferred, though not a requirement

  • Excellent Follow-up Skills

  • Possess the ability to effectively read, Write, and communicate in English

  • Able to work flexible hours as needed, and respond during off-duty hours in times of emergencies when all team members may be needed

  • Ability to manage multiple priorities

  • Ability to use the computer

  • Detail Oriented

  • Report Daily

  • Demonstrates organizational skills and ability to coordinate and plan
