Physical Therapist – SNF Facility (Los Gatos, CA 95032) (Contract)

Schedule:

• 13-week contract (ASAP start)

• 36 hours/week

• Travel assignment

Pay:

• $85–$90/hour

Position Summary & Responsibilities:

• Evaluate and treat SNF residents

• Develop and implement individualized PT treatment plans

• Provide gait training, therapeutic exercise, and mobility support

• Document patient evaluations and progress

• Collaborate with interdisciplinary care team

• Support functional recovery and discharge planning
