Job Description
Fifth Star Care Management is seeking a dedicated and compassionate Occupational Therapist (OT) to join our IN-HOUSE therapy team! New Grads Welcome!
Full-Time Split Position between Three Rivers Health & Rehab and Northern Neck Senior Care Community, with a $2,500 sign-on bonus.
Key Responsibilities:
Conduct comprehensive evaluations of residents' physical, cognitive, and psychosocial abilities.
Develop and implement personalized treatment plans that promote independence in activities of daily living (ADLs), functional mobility, and leisure activities.
Provide direct therapy services in accordance with physician orders and established care plans.
Maintain accurate, timely documentation of evaluations, treatment plans, progress notes, and discharge summaries.
Participate in interdisciplinary team meetings and contribute to care planning processes.
Educate residents, families, and staff on adaptive equipment, home safety, and strategies to support functional independence.
Supervise occupational therapy assistants (COTAs) and ensure compliance with professional and regulatory standards.
Adhere to all applicable regulations, ethical standards, and company policies.
Qualifications:
Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
Current and valid license to practice Occupational Therapy in the state of Virginia.
Certification by the National Board for Certification in Occupational Therapy (NBCOT).
Previous experience in a long-term care or geriatric setting preferred.
Excellent interpersonal and communication skills.
Strong organizational and time-management abilities.
