Fifth Star Care Management is seeking a dedicated and compassionate Occupational Therapist (OT) to join our IN-HOUSE therapy team! New Grads Welcome!

Full-Time Split Position between Three Rivers Health & Rehab and Northern Neck Senior Care Community, with a $2,500 sign-on bonus.

Key Responsibilities:

  • Conduct comprehensive evaluations of residents' physical, cognitive, and psychosocial abilities.

  • Develop and implement personalized treatment plans that promote independence in activities of daily living (ADLs), functional mobility, and leisure activities.

  • Provide direct therapy services in accordance with physician orders and established care plans.

  • Maintain accurate, timely documentation of evaluations, treatment plans, progress notes, and discharge summaries.

  • Participate in interdisciplinary team meetings and contribute to care planning processes.

  • Educate residents, families, and staff on adaptive equipment, home safety, and strategies to support functional independence.

  • Supervise occupational therapy assistants (COTAs) and ensure compliance with professional and regulatory standards.

  • Adhere to all applicable regulations, ethical standards, and company policies.

Qualifications:

  • Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.

  • Current and valid license to practice Occupational Therapy in the state of Virginia.

  • Certification by the National Board for Certification in Occupational Therapy (NBCOT).

  • Previous experience in a long-term care or geriatric setting preferred.

  • Excellent interpersonal and communication skills.

  • Strong organizational and time-management abilities.
