Position Summary

The Licensed Physical Therapist is responsible for evaluating and treating residents, developing individualized treatment plans, and providing high-quality rehabilitative services. This role collaborates closely with physicians, nursing staff, and other therapy professionals to maximize resident outcomes while ensuring compliance with all regulatory and documentation requirements.

Key ResponsibilitiesResident Care & Evaluation
  • Evaluate residents based on physician referrals and orders.

  • Develop individualized treatment plans with measurable goals.

  • Provide skilled physical therapy treatments to improve mobility, function, and quality of life.

  • Educate residents and families regarding treatment plans, exercises, and discharge recommendations.

  • Participate in discharge planning to ensure continuity of care.

Documentation & Compliance
  • Maintain accurate, timely, and complete clinical documentation.

  • Record daily treatment services in accordance with billing and reimbursement guidelines.

  • Ensure compliance with Medicare, Medicaid, and other regulatory requirements.

  • Stay current on documentation standards and reimbursement regulations.

Collaboration & Communication
  • Work collaboratively with physicians, nursing staff, and the interdisciplinary care team.

  • Participate in care conferences, team meetings, and quality improvement initiatives.

  • Communicate resident progress and treatment recommendations with families and healthcare providers.

Leadership & Professional Development
  • Supervise Physical Therapist Assistants (PTAs), rehabilitation technicians, therapy students, and provisional licensees as appropriate.

  • Provide education and in-service training on restorative therapy techniques.

  • Mentor and support new team members to promote clinical excellence.

Department Support
  • Maintain a clean, organized, and safe therapy environment.

  • Ensure therapy equipment is functioning properly and report maintenance concerns.

  • Assist with departmental projects and quality initiatives as assigned.

  • Travel or provide support to affiliated facilities when needed.

Working Conditions
  • Active work environment requiring standing, walking, bending, lifting, pushing, and pulling throughout the workday.

  • Frequent interaction with residents, families, physicians, and interdisciplinary team members.

  • Ability to respond appropriately in fast-paced and changing clinical situations.

  • Potential exposure to bloodborne pathogens and infectious diseases while following established infection prevention protocols.

QualificationsEducation & Licensure
  • Graduate of an accredited Physical Therapy program.

  • Current Physical Therapist license (or eligibility for licensure) in the state of practice.

Experience & Skills
  • Previous experience in skilled nursing, rehabilitation, or long-term care preferred but not required.

  • Strong clinical assessment and problem-solving skills.

  • Excellent communication, organization, and time management abilities.

  • Ability to work independently while contributing effectively to a collaborative team.

Physical Requirements
  • Ability to lift up to 50 pounds and assist with resident mobility and transfers.

  • Ability to stand, walk, bend, stoop, reach, push, and pull throughout the workday.

  • Ability to maintain professionalism
