Case Management coordinator
resident financial services and coordinates medical/financial transportation. Particularly, the role interacts with resident physicians, specialists, consultants, healthcare providers, and any other ancillary medical financial services needed by the residents. Case Management coordinator is the first point of contact for resident and family financial concerns.
Job duties include:
Refer and schedule residents to see the in-house doctor or their own physician on an annual and as needed basis and assure proper documentation.
Refer residents to see the outside doctor, clinic, and specialists as referred by doctor.
SSI/SSP Assistance
Respond to Medicaid waiver letters appropriately.
Financial Applications
Enter/Retrieve resident information on the computer system.
Assist with admissions, room moves and hospital returns
Prepare the daily census report
Position Requirements:
Must be able to communicate effectively utilizing oral, written, and technical modes of communication.
Must be computer literate and capable of utilizing word processing.
Must be able to lead by example as demonstrated by consistently timeliness, task delegation, constant follow up, offering direction, and always available to clients and employees alike in a supporting role.
Must be very organized and able to complete standard office tasks with minimal assistance.
Location is on site, some schedule flexibility is preferable.
Pre-employment health screening and background check required.

Salary for a Case Management Benifit's Coordinator: $18-20/hr
