In this role, you will provide compassionate, resident-centered nursing support in an assisted living and memory care environment. You will administer medications and treatments, monitor residents for changes in condition, maintain accurate documentation, and work closely with residents, families, providers, pharmacies, and community team members.

What You'll Do

  • Administer medications and treatments according to provider orders, RN direction, applicable nursing standards, and community procedures
  • Monitor residents for changes in physical condition, cognition, behavior, mood, appetite, mobility, and daily functioning
  • Obtain and document vital signs and other assigned resident health information
  • Provide direct nursing care and resident support within the New Jersey LPN scope of practice
  • Collect, document, and communicate resident information to the supervising RN or appropriate nursing leader
  • Promptly report significant changes in resident condition
  • Carry out nursing interventions and treatments established by the RN or provider
  • Participate in resident assessments and service-plan reviews under RN direction
  • Assist with implementing and maintaining individualized resident service plans
  • Receive, review, and carry out provider orders within the LPN’s scope of practice and community procedures
  • Coordinate medication refills and communicate with pharmacies and providers
  • Maintain accurate electronic and written documentation of medications, treatments, observations, incidents, and nursing services
  • Perform medication-cart, treatment-record, and documentation audits as assigned
  • Support resident admissions, hospital returns, transitions, and changes in condition
  • Provide resident and family education within the LPN’s training, competency, and scope of practice
  • Promote hydration, nutrition, infection prevention, medication safety, fall prevention, and overall wellness
  • Respond promptly to resident emergencies and urgent needs
  • Communicate with families and providers regarding resident needs and changes as directed
  • Participate in resident-care meetings and Wellness team communication
  • Provide day-to-day guidance and support to resident-care team members
  • Reinforce resident service plans, care instructions, and community procedures
  • Assist with staff training, competency reviews, audits, and in-service education
  • Maintain resident privacy, dignity, rights, and confidentiality
  • Complete additional nursing and Wellness duties within the LPN’s lawful scope of practice

What We're Looking For

  • Current and valid New Jersey LPN license
  • License must be active and in good standing
  • Knowledge of practical nursing standards, medication administration, infection prevention, and resident safety
  • Strong observation and resident-monitoring skills
  • Ability to recognize, document, and report changes in resident condition
  • Ability to work under RN direction and seek guidance when nursing assessment or independent nursing judgment is required
  • Strong documentation, organization, and time-management skills
  • Comfortable using electronic health records or resident-care documentation systems
  • Ability to communicate professionally and compassionately with residents, families, providers, pharmacies, and coworkers
  • Ability to communicate clearly and effectively in English, verbally and in writing, as required to understand provider orders, document nursing services, administer medications safely, report changes in condition, and respond during emergencies
  • Ability to provide appropriate direction and support to resident-care staff
  • Ability to work independently within the assigned role while recognizing when a concern must be escalated
  • Current CPR or Basic Life Support certification, if required by the community
  • Previous assisted living, memory care, long-term care, home care, or senior-care nursing experience is preferred
  • Previous experience working with older adults is preferred
  • Dependable attendance and punctuality
  • Ability to stand, walk, bend, reach, and remain active throughout the workday
  • Ability to safely lift, transfer, push, or pull up to (physical requirement) pounds with assistance or available equipment
  • Ability to successfully complete required background checks, health screenings, and employment requirements

New Jersey Nursing Requirements

  • Must maintain an active New Jersey LPN license throughout employment
  • All nursing services must be performed within the New Jersey LPN scope of practice
  • Initial nursing assessments and responsibilities requiring independent professional nursing judgment must be completed by a Registered Nurse
  • The LPN may collect resident information, contribute to care planning, implement assigned nursing interventions, and report resident outcomes under RN direction
  • The LPN may not independently establish nursing diagnoses or perform responsibilities reserved for an RN
  • Nursing duties must be consistent with the resident’s condition, the LPN’s education and competency, RN direction, provider orders, and community procedures
  • Delegation of nursing tasks to unlicensed resident-care staff must be completed by the RN when required under applicable New Jersey standards
  • The LPN may provide direction, observation, reinforcement, and follow-up for assigned resident-care duties
  • Significant changes, complex conditions, or duties outside the LPN’s scope must be promptly escalated to the supervising RN, provider, or emergency services
  • Medication administration and documentation must comply with provider orders, applicable New Jersey requirements, and community procedures
