Job Title:Licensed Practical Nurse (LPN)Reports to:Patient Care CoordinatorFLSA Status:Non-exemptPosition Scope:The LPN provides direct care under the supervision of the RN Team Manager. The LPN is responsible for following the plan of care, providing treatment and working in conjunction with the members of the team in order to ensure comfort and quality of life.Essential Functions:Provides routine nursing visits and personal care while following the plan of care:Monitors effectiveness of the plan of care in controlling pain and other symptomsObserves, collects, records and determines the need for more dataAssists with nursing interventions to resolve current problems or prevent potential problemsReport's findings to the RN team Manager and in the interdisciplinary meetingsProvides comfort oriented physical care to patients:Provides wound care and changes dressings as neededInserts and irrigates Foley catheter as orderedAdministers enemas and removes impaction as orderedTube feedingsPCATakes and records vital signsAssists with daily living activitiesInitiates change in patient's level of care as appropriate to meet the needs of the patient and/or familyProvides continuous care nursing for the patient as directed in the patient's plan of careProvides and documents teaching to patients and/or families to assist in the patient's comfort and records the response of the family and/ or familyPractices and role models universal precautions in all contacts with patientsDemonstrates the physical care needed for the patients, family or Hospice AideProvides emotional support to the patient and/or their families in a manner that fosters independence and affirms existing strengthsComplies with the Privacy Policies of the organization to ensure compliance with the Health Insurance Portability and Accountability ActRepresents and maintains the philosophy of the organizationPerforms other work duties as assignedSupervisor Responsibilities:NoneKnowledge and Skills:Understanding of the various regulatory guidelines (ethical, confidentiality, legal requirements, etc.)Demonstrates a working knowledge of the stages of uncomplicated grief and the dying processPossesses basic physical assessment skillsPossesses the interpersonal skills that promote interdisciplinary collaboration and effective communicationEducation and Work Experience:Minimum of 1-year patient care experience as a licensed practical nurse; hospice or home health preferredValid driver's license and automobile insurance, and reliable transportationCertification and Licenses:Licensure in West Virginia and OhioWork Environment and/or Physical Demands:The LPN sits, stands, bends, pushes and pulls, drives and lifts intermittently throughout the day. TheLPN must have sufficient strength to turn, lift, and assist patients.Confidentiality:The LPN must carry out responsibilities in an ethical, professional and confidential manner.Employment is contingent upon successful completion of a background check, drug screening, and possession of a valid driver's license.
