Home Care - Care Coordinator/Admin

Seva Senior Services is currently seeking a compassionate, organized, and dependable Care Coordinator to join our growing team.

This role combines caregiver support, client coordination and compliance tracking. The Care Coordinator serves as a key connection between caregivers, clients, clinical leadership, and internal operations to help ensure clients continue receiving high-quality care and support services.

We are looking for someone who enjoys working with people, building relationships, staying organized, and supporting families in the community.

Responsibilities
  • Case Management communication for services changes and updates regarding schedules, travel, incidents on a daily basis

  • New service starts for home care/structured family care including orientation, app training, scheduling (match with authorization), and client DocuSign

  • Updating client records in the EHR and CRM

  • Client home visits and assessments

  • Communicating with the team regarding updates to any services

  • Organizing and filing all documentation digitally in the client record folders

  • Provide caregiver support, education, and guidance

  • Manage client’s clock-in and clock-out schedule in the system

  • Review electronic caregiver logs and documentation for compliance

  • Track documentation, authorizations, and required follow-ups

  • Support ongoing audit readiness and electronic file accuracy

  • Collaborate closely with nursing leadership and internal departments

  • Assist caregivers with system navigation and documentation questions

  • Report urgent client concerns or incidents appropriately

  • Help maintain timely communication regarding client updates and care needs

  • Any other duties as assigned within the care coordinator role

Qualifications
  • 2+ years of experience in healthcare, home care, caregiving support, healthcare administration, case management, or a related field preferred

  • Experience working with Medicaid waiver programs, home care services, or community-based healthcare programs is preferred

  • Strong communication and relationship-building skills

  • Strong organizational skills and attention to detail

  • Comfortable using electronic healthcare systems, documentation platforms, or EVV systems

  • Ability to manage multiple responsibilities in a fast-paced environment
