Position Summary & Responsibilities (SNF)
Evaluate and treat SNF residents in assigned specialty area (Ortho, Pelvic Floor, or Lymphedema)
Provide skilled therapy services and documentation
Ensure Medicare/PDPM compliance
Collaborate with interdisciplinary care team
Maintain productivity and quality standards
Requirements
Active Physical Therapist (PT) license
SNF or post-acute rehab experience required
Specialty experience in Ortho, Pelvic Floor, or Lymphedema preferred
Knowledge of Medicare/PDPM and documentation standards
Strong communication and teamwork skills
