Responsibilities:

Develop and implement individualized physical therapy plans to achieve optimal patient outcomes
Provide skilled physical therapy interventions, including exercises, modalities, and other treatments
Collaborate with the interdisciplinary team to ensure comprehensive care and achieve patient goals
Effectively communicate with patients, families, and healthcare professionals to ensure seamless care transitions
Maintain accurate and timely documentation of patient progress and treatment plans
