Salary Range: $32.54 – $36.46 USD

Responsibilities
- Personalized Patient Care: Evaluate, plan, and implement occupational therapy treatments tailored to individual patient needs in a home setting.
- Interdisciplinary Collaboration: Work closely with specialized nurses, social workers, dieticians, and aides to ensure a comprehensive continuum of care.
- Documentation: Maintain accurate and timely clinical records, ensuring all assessments and treatment plans meet regulatory and agency standards.
- Professional Development: Assume responsibility for ongoing continuing education and participate in process improvement opportunities within the agency.
- Patient Advocacy: Advise and consult with family members, physicians, and other team members to ensure optimal patient outcomes.
- Travel & Logistics: Manage a traveling schedule to see patients throughout the designated service area, with mileage reimbursement provided at the federal rate.
Minimum Essential Requirements
- Graduate of an accredited Occupational Therapy curriculum (AMA/AOTA accredited).
- Current Occupational Therapist (OT) licensure.
- One (1) year of Occupational Therapy experience to practice without supervision (or less than one year if practicing under specific supervision guidelines).
- Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
- Valid Driver’s License and Automotive Insurance.
- Ability to clear all required background checks (Act 33, Act 34, OAPSA).
