Job Summary:

As a Physical Therapist Home Health, you will evaluate patients in their homes, develop individualized treatment plans, and provide skilled physical therapy services. You will educate patients and caregivers on exercises, safety, and mobility, and collaborate with physicians, nurses, and other healthcare professionals to promote safe, high-quality patient care and excellent outcomes.

Responsibilities:

• Evaluate patients in their homes and develop individualized treatment plans

• Provide skilled physical therapy services

• Educate patients and caregivers on exercises, safety, and mobility

• Complete OASIS and clinical documentation in a timely manner

• Collaborate with physicians, nurses, and other healthcare professionals

• Monitor patient progress and adjust treatment plans as needed

• Promote safe, high-quality patient care and excellent outcomes

Qualifications:

• Graduate of an accredited Physical Therapy program

• Current Michigan Physical Therapist license

• Current CPR certification

• Minimum of 2 years of PT experience preferred

• Home Health experience is a plus but not required

• Valid driver's license, reliable transportation, and auto insurance
