Job Summary:

We are seeking a highly skilled and motivated Physical Therapist to join our team. As a Physical Therapist, you will provide physical therapy services to our patients, promoting excellence of rehab care, promoting interdisciplinary cooperation, and quality patient care. Responsibilities:

• Evaluate patients within timely fashion, as required by state, payor source, or facility requirements
• Provide physical therapy services to patients, including evaluation, treatment planning, and implementation
• Communicate with other disciplines and interacts with staff as necessary, as indicated by the treatment plan
• Provide routine documentation according to Medicare, facility, and other payor source guidelines
• Participate in family conferences when requested
• Record and submit in timely fashion all required documentation and treatment charges, according to policy and procedure

Qualifications:

• Current MA State License as a Physical Therapist
• Must have flexibility as a person to be a part of a dynamic and cooperative team, to distribute the workload equitably and according to the individual's staff position within the department
• Have the capability to evaluate or assess patients, participate in consultations, teaching and meeting with all levels of residents, staff, family, and other professionals, in order to plan individualized treatment plans and goals, discuss and plan appropriate discharge plans, and teach and counsel patients and staff
