Duties and Responsibilities
%CF; Provide total nursing care to residents on the assigned unit during the shift as a Licensed Practical Nurse (LPN)
%CF; Participate in developing and implementing individualized care plans alongside the healthcare team
%CF; Administer medications and treatments as prescribed and evaluate effectiveness regularly
%CF; Maintain an environment that promotes residents’ physical, social, and psychological well-being
%CF; Educate residents and families on care plans and medical procedures
%CF; Monitor team member performance and assist with development, evaluation, and corrective action if needed
%CF; Delegate and ensure completion of assigned nursing tasks to team members
%CF; Stay current with facility policies, procedures, and ongoing nursing education
%CF; Recognize and act on significant changes in resident conditions
%CF; Document care, responses, and relevant information per facility guidelines
%CF; Act as a charge nurse: answer questions, resolve issues, and support resident care decisions
%CF; Follow all facility safety policies and procedures
Skills and Qualifications
%CF; Graduation from an accredited nursing education program
%CF; Active Licensed Practical Nurse (LPN) license in the state of Illinois
%CF; No prior abuse convictions
%CF; Strong communication and teamwork skills
%CF; Ability to follow policies, maintain safety, and deliver empathetic care
%CF; Must be able to work with diverse resident populations, including elderly and mentally ill individuals
%CF; Must be physically able to perform nursing duties with or without accommodations
