Position SummaryThe Speech Language Pathologist (SLP) evaluates, diagnoses, and treats residents with speech, language, cognitive-communication, and swallowing disorders. This role collaborates closely with physicians, nursing staff, rehabilitation professionals, residents, and families to develop individualized treatment plans that promote the highest level of independence and quality of life.
What You'll DoEvaluate and treat residents with speech, language, cognitive-communication, and swallowing disorders.
Develop and implement individualized treatment plans based on each resident's goals and needs.
Monitor resident progress and modify treatment plans as appropriate.
Collaborate with physicians, nursing staff, therapists, and the interdisciplinary care team.
Educate residents, families, and caregivers on therapy techniques and communication strategies.
Maintain accurate and timely clinical documentation in accordance with regulatory and professional standards.
Participate in interdisciplinary care planning meetings.
Ensure compliance with Medicare, Medicaid, and facility documentation requirements.
Support quality improvement initiatives and contribute to positive resident outcomes.
Master's Degree in Speech-Language Pathology.
Current state licensure as a Speech Language Pathologist.
Certificate of Clinical Competence (CCC-SLP) preferred but not required.
Experience in skilled nursing or long-term care is preferred.
Strong communication, organization, and clinical assessment skills.
Passion for serving the geriatric population.
