Job Description
We are seeking a compassionate and skilled Physical Therapist to join our growing rehabilitation team at our Bayville location. We are hiring Full-Time or PRN PTs to provide high-quality care to residents in our skilled nursing and post-acute rehab programs.

In-house skilled therapy services with hands-on patient care
As a Physical Therapist, you will evaluate and treat residents to improve mobility, strength, and overall quality of life. You will work closely with nurses, physicians, and other rehab staff to create and implement individualized treatment plans, ensuring residents receive the best care possible.
Key ResponsibilitiesConduct comprehensive evaluations to determine therapy needs
Develop and implement individualized treatment plans
Provide therapy interventions to improve mobility, function, and independence
Educate residents and families on exercises and treatment plans
Document progress and adjust therapy plans as needed
Collaborate with healthcare team for care coordination and discharge planning
Ensure compliance with CMS and facility documentation requirements in NetHealth EMR
Maintain knowledge of evidence-based best practices in physical therapy
Associates Degree or higher in Physical Therapy from an accredited program
Active New Jersey Physical Therapy license
Ongoing commitment to continuing education and professional development
Long-term care experience is a plus
Strong organizational, communication, and patient care skills
