Smoky Ridge Health and Rehabilitation is seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team in a skilled nursing facility. The ideal candidate will have a passion for helping patients regain their strength, mobility, and independence following illness, injury, or surgery. This role involves evaluating patients, developing personalized treatment plans, and delivering high-quality, evidence-based therapy services to residents.

Key Responsibilities:
  • Perform comprehensive physical therapy evaluations and re-assessments

  • Develop individualized treatment plans based on patient goals and clinical findings

  • Provide direct physical therapy services in accordance with physician orders and facility protocols

  • Monitor and document patient progress, outcomes, and response to treatment

  • Collaborate closely with interdisciplinary team members including occupational therapists, speech-language pathologists, nurses, and physicians

  • Participate in care plan meetings and family conferences as needed

  • Educate patients, caregivers, and staff on exercise programs and mobility techniques

  • Maintain accurate and timely documentation in the electronic medical record system

  • Ensure compliance with all federal, state, and facility regulations and guidelines

  • Supervise and mentor Physical Therapy Assistants (PTAs) and support staff when appropriate

Qualifications:
  • Graduate of an accredited Physical Therapy program

  • Current state licensure as a Physical Therapist (or eligible)

  • CPR certification (BLS) required

  • Experience in a SNF or geriatric setting preferred but not required

  • Excellent communication, organizational, and interpersonal skills

  • Strong clinical reasoning and documentation skills

  • Ability to work effectively as part of a collaborative team.

Work Schedule:
  • As needed with occasional weekend or holiday coverage as requested.
