THE NORTHWARD CENTER
TITLE: LICENSED PRACTICAL NURSE
REPORT TO: DIRECTOR OF NURSING
SUMMARY DESCRIPTION
The incumbent, under the supervision of DON, provides skilled nursing care to the clients. The incumbent ensures that all work conforms to company policies and procedures, safety and regulatory laws and standards.
DUTIES AND RESPONSIBILITIES
- Provide nursing care according to the individual plan of care, physician's order and emergency Ensure the accuracy and completeness of medical record documentation.
- Performs client assessment and communicate with DON in developing and implementing client's plan of
- Coordinate with other internal and external ancillary services (i.e.: Pharmacy, Dental, Rehabilitation, PT, OT, Speech Therapy) to ensure quality client Communicates with physicians to provide follow up care.
- Maintain nursing station; ensure that stock medications and emergency items are in
- Maintains established center's policies and procedures, objectives, quality assurance program, safety and infection control
- Maintains and respects client's and employee's confidentiality. Promotes adherence to the Client's Bill of
- Perforn1s duties of Licensed Practical

POSITION SUPERVISED
Directly: Certified Nursing assistant
QUALIFICATIONS
Education: LPN with New Jersey License
CPR certification an valid malpractice insurance policy.
Experience: 1-2 year in a licensed health care facility. Geriatric and long-term care experience preferred. Bilingual (English/ Spanish) preferred.
KNOWLEDGE, SKILLS, AND ABLITIES
Knowledge of company policies and procedures.
Knowledge of aging process to the illness and ages of the clients served. Knowledge of NJ Medicaid Service Manual for Medical Day Care services.
Knowledge of NJ Dept. of Health Manual of Standards for adult Day Health Care Licensure.
Knowledge of applicable state and federal environmental and OSHA regulations.
Ability to demonstrate excellent English communication skills, both orally and in writing. Ability to demonstrate excellent interpersonal skills.
Ability to demonstrate excellent organizational skills. Ability to handle multiple priorities.
Ability to work in a cross-functional capacity. Ability to work flexible schedules on demand.
