Physical Therapist (PT)

We are currently seeking a Physical Therapist (PT).

• Responsible for the assessment of referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation.
• Establish and administer treatment programs with specific goals determined according to each patient’s capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients.
• Document and record patients’ conditions and educate patients and families in appropriate physical therapy methods.
• Complete evaluations, weekly progress notes, recertification, and discharge documentation.
• Collaborate with all disciplines to provide the best patient-centered treatment and care.
• Perform all other duties assigned by the Director of Rehabilitation.
• Ensure compliance with confidentiality, HIPAA, and Federal Residents’ Rights.

• Graduation from an accredited Physical Therapy college or university.
• Valid professional license and registration for the state of practice.
• Experience with long-term care and/or geriatric care is preferred but not required.
• Experience with Case Mix Index and PPS scheduling is preferred but not required.
