As an Evening Shift Licensed Practical Nurse, you will play a vital role in providing exceptional care to our residents while promoting a positive and supportive environment. About the Role:
Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)
Develop and implement individualized care plans in collaboration with the healthcare team
Monitor and report changes in patient condition to the healthcare team
Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care
Maintain accurate and up-to-date patient records, reports, and documentation
Responsibilities:
Work closely with the evening shift nursing team to ensure a smooth transition of patient care
Participate in unit-based quality improvement initiatives to enhance patient outcomes and satisfaction
Maintain a safe and clean environment, adhering to Pearl Pavilion's infection control and safety policies
Participate in in-service training and continuing education programs to stay current with industry best practices and regulations
Demonstrate compassion, empathy, and respect for all patients, families, and colleagues
