Job Summary:

The Care Coordinator is responsible for leading care coordination efforts across the center for all patients. This position provides patient navigation, referral management, and coordination of care, ensuring timely access to services, effective follow-up, and closure of care gaps. The Care Coordinator collaborates with interdisciplinary teams to support high-risk patients, improve transitions of care, and strengthen the continuity of care across the organization and community.

Key Responsibilities:

Care Coordination

  1. Provide comprehensive care coordination for all patients, with prioritization of high-risk and high-need populations.
  2. Conduct patient outreach, intake, and needs assessment including Social Determinants of Health (SDOH).
  3. Develop and implement individual care plans in collaboration with providers and care teams.
  4. Coordinate services across primary care, behavioral health, dental, WIC, specialty care, and community resources.
  5. Support transitions of care, including hospital and emergency room follow-up.
  6. Ensure referrals are completed from initiation to closure, ensuring timely follow-up and documentation.
  7. Assist patient with appointment coordination and access to services.
  8. Promote continuity of care by addressing care gaps and supporting patient engagement.

Qualifications:

  • Highschool diploma preferred
  • Minimum 5-10 years in healthcare or social services related field preferred
  • Experience in administrative and client services preferred
  • Ability to speak, read, writes and understands English
  • Strong verbal and written communication skills, as well as computer literacy
  • Proven reliability, trustworthiness, flexibility, and high ethical standards
  • Ability to problem solve and handle multiple tasks
  • Professional, punctual, maintains regular attendance, must be team oriented & co-operative
  • Demonstrated ability to treat all individuals with respect and aloha
