Speech Language Pathologist (FULL TIME OR PRN)
Heritage Grove Estates is hiring a Full-Time or PRN SLP to join our in-house rehab team in Topeka. We’re looking for someone who values patient-centered care and thrives in a team-oriented environment. New grads are encouraged to apply!
$5,000 Sign-On Bonus Available
Key Responsibilities
• Provide speech-language pathology services to address communication, cognition, and swallowing disorders
• Deliver skilled therapy services in accordance with an established plan of care
• Evaluate, treat, and modify individualized care plans based on resident progress
• Assess and document residents’ speech, language, cognitive, and swallowing function
• Educate residents, families, and caregivers on therapy goals, strategies, and safe swallowing techniques
• Maintain accurate, timely documentation in compliance with company policies and state regulations
• Communicate resident progress and clinical observations with the interdisciplinary team
• Participate in care planning meetings and collaborate with nursing, therapy, and medical staff
Qualifications
• Active Kansas Speech-Language Pathologist (SLP) license
• Certificate of Clinical Competence (CCC-SLP) preferred
• In good standing with all applicable licensing boards
• Experience in skilled nursing or long-term care settings preferred
• Familiarity with Net Health documentation system is a plus
• Strong communication and interpersonal skills
