Phoenix of Maple Heights is seeking a PRN Physical Therapist (PT) to join our in-house rehab team in a skilled nursing and long-term care setting.

Position Highlights:

• Flexible PRN schedule

• In-house rehab program

• SNF/LTC environment

• Collaborative interdisciplinary team

• Documentation systems: Net Health & PointClickCare (PCC)

Qualifications:

• Current Ohio PT license

• Strong clinical and communication skills

• SNF/LTC experience preferred, but not required
