About the Role
As a Home Health Physical Therapist with Freedom at Home, you will:
Evaluate patients in their homes and develop individualized treatment plans
Provide skilled physical therapy services
Educate patients and caregivers on exercises, safety, and mobility
Complete OASIS and clinical documentation in a timely manner
Collaborate with physicians, nurses, and other healthcare professionals
Monitor patient progress and adjust treatment plans as needed
Promote safe, high-quality patient care and excellent outcomes

Competitive pay: $55 - $65 HOURLY

Requirements
Graduate of an accredited Physical Therapy program
Current Michigan Physical Therapist license
Current CPR certification
Minimum of 1 year of PT experience
Home Health experience is a plus but not required
Valid driver's license, reliable transportation, and auto insurance
