Home Health Physical Therapist
Access TLC Home Health Care is currently seeking professional Physical Therapists in Simi Valley, Thousand Oaks and surrounding areas
Position is per diem/per visit. Visit rates range from $100 - $130 per visit.
Our agency offers a unique program designed to support patients at home. Our team of professionals are experts in providing home health care services. We have 22 years of experience and run a well-established organization!
Requirements:
- Current California PT License
- Valid California Driver's License with proof of a good Motor Vehicle Record
- Current proof of auto insurance with state minimum (or greater) coverage
- Social Security Card and/or proof of eligibility to work in U.S.
- Two Professional References
- Current CPR Certification
