Lake Wales Care Center

Full-Time 40 hours/ and PRN Opportunities Available

$5000.00 SIGN ON/RELOCATION BONUS available for direct hire.

About the Role:
We are seeking a compassionate, motivated, and clinically skilled Physical Therapy Assistant (PTA) to join our rehabilitation team at Lake Wales Care Center located in Lake Wales, FL. As a Physical Therapy Assistant at Lake Wales, you will play a key role in restoring mobility, improving functional independence, and enhancing the overall quality of life for our residents. This physical therapy assistant position is open position is a growth position due to census. It is a great opportunity to join a dynamic team of therapists and assistants.

Physical Therapy Assistant Responsibilities:

  • Provide feedback to evaluating therapist regarding residents functional needs and recommendations to their individualized treatment plans.

  • Provide high-quality physical therapy services including therapeutic exercises, mobility training, neuromuscular re-education, and wound prevention strategies.

  • Collaborate with an interdisciplinary team including OT, SLP, nursing, and physician partners.

  • Document screenings, progress notes, and objective data towards discharge summaries in accordance with regulatory and facility standards.

  • Participate in care plan meetings and contribute to overall resident-centered care.

  • Educate residents, families, and caregivers on safe mobility techniques and home exercise programs.

  • Maintain compliance with state and federal guidelines, including Medicare and SNF regulatory requirements.

Physical Therapy Assistant Qualifications:

  • Licensed Physical Therapy Assistant (PTA) in the state of Florida (or eligibility for licensure).

  • Graduate of an accredited Physical Therapy Assisting program (DPT or equivalent).

  • Previous SNF experience preferred, but new grads are welcome to apply.

  • Strong communication, organizational, time management,team player, and clinical reasoning skills.

  • Commitment to delivering exceptional patient-centered care with great outcomes.
