Part-Time or Per Diem Physical Therapist (PT/PTA) - Home Health Visits

We are looking for a compassionate, reliable PT and PTA to join our team to provide excellent care and who enjoys working with patients in an at-home setting.

Responsibilities:

  • Provides care and services in accordance with the plan of care.

  • Assists community members and health field personnel to assess, plan for, and provide needed health and related services.

  • Documents care provided in an accurate and timely manner.

  • Provides teaching to clients and families.

  • Coordinates and collaborates with other members of the health care team, community agencies, and referral sources.

  • Communicates the plan of care with patients and families and insures understanding of the exercise program prescribed.

  • Responsive to meeting or exceeding patient/client expectations and promoting a positive image of the company

  • Adheres to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.

  • Attends continuing education programs.

  • Other duties assigned by the department manager.

Requirements/Qualifications:

  • Associate's degree and graduation from an accredited Physical Therapy program.

  • One year of physical therapy experience preferred.

  • Possesses and maintains CPR Certification

  • Possess valid driver's license

Skills/Competencies:

  • Demonstrates responsibility for self-direction

  • Demonstrates the ability to learn and utilize computer technology

  • Effective communication skills

  • The ability to work in a team-oriented environment as well as independently


Qualifications:

Must be licensed by the State of Massachusetts as a Physical Therapy Assistant.

Job Types: Part-time, Per-diem

Expected hours: 10 – 30 hours per week
