Location: Nebo, North Carolina
Job Summary:
We are seeking a compassionate and motivated Physical Therapist (PT) to join our therapy department at Deer Park Health & Rehabilitation. As a Physical Therapist PRN, you will have the opportunity to evaluate, plan, and provide skilled interventions to help patients regain independence, improve mobility, and enhance overall quality of life. Physical Therapist Responsibilities:
Evaluate patients and develop individualized treatment plans based on clinical assessments
Provide direct physical therapy services in accordance with physician orders and facility policies
Supervise and collaborate with Physical Therapist Assistants (PTAs) to ensure continuity of care
Document evaluations, treatments, and patient progress accurately and timely
Collaborate with the in-house therapy team, nursing, and interdisciplinary staff to coordinate care
Educate patients, families, and caregivers on therapeutic interventions and home exercise programs
Physical Therapist Qualifications:
Graduate of an accredited Physical Therapy program
Current state licensure as a Physical Therapist in NC
Strong interpersonal, clinical, and documentation skills
Ability to work collaboratively as part of a fully integrated, in-house therapy team
Experience in a skilled nursing facility or rehabilitation setting preferred, but not required
