Position Summary:
We are currently seeking a Per Diem Physical Therapist (PT) to join our in-house rehabilitation team. As a PT, you will assess, plan, and provide physical therapy services to patients recovering from illness, injury, or surgery, with the goal of maximizing function and quality of life.

Key Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical findings and functional goals.

• Provide direct physical therapy services to improve mobility, strength, balance, and overall physical function.

• Collaborate with Physical Therapist Assistants (PTAs) and other members of the rehab and nursing teams to coordinate care.

• Monitor and document patient progress accurately and timely in accordance with regulatory and facility standards.

• Educate patients, families, and caregivers on treatment plans, home exercises, and safety techniques.

• Ensure compliance with all state, federal, and facility regulations.

Qualifications:

• Graduate of an accredited Physical Therapy program.

• Current licensure as a Physical Therapist in the state of Florida.

• Experience in a skilled nursing or subacute rehab setting preferred but not required.

• Strong clinical reasoning, documentation, and communication skills.

• Flexible, dependable, and committed to providing high-quality patient care.
