Responsibilities:
Provide direct patient care, including assistance with daily living activities, vital signs monitoring, and mobility exercises
Develop and maintain effective relationships with residents, families, and healthcare teams
Collaborate with licensed nurses to develop and implement individualized care plans
Maintain accurate and detailed records of resident care
Participate in quality improvement initiatives and contribute to a positive and respectful work environment
