Chatham Glen Healthcare & Rehab Center, located in the heart of The Villages, FL, is currently seeking a dedicated Physical Therapist (PT) to join our in-house Subacute Rehabilitation team. We proudly deliver high-quality, patient-focused care through a fully integrated therapy model — no contract providers.

Key Responsibilities:

• Evaluate and treat patients with physical impairments, functional limitations, and disabilities
• Develop individualized treatment plans with measurable, outcome-focused goals
• Collaborate with an interdisciplinary team including occupational therapy, speech therapy, nursing, and medical staff
• Document all services provided, track patient progress, and adjust treatment plans as needed
• Educate patients, families, and caregivers on home exercise programs and rehabilitation goals

Qualifications:

• Licensed Physical Therapist in Florida (or eligible for licensure)
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Prior experience in a skilled nursing or subacute rehab setting is a plus
• Strong communication, clinical reasoning, and time management skills
