About the Role:
As an Occupational Therapist at Hope Creek Nursing and Rehabilitation Center, you will play a vital role in helping our residents achieve their maximum physical, mental, and functional abilities. You will work closely with our multidisciplinary team to develop and implement personalized therapy plans, focusing on activities of daily living, to enhance our residents' quality of life. Your expertise will enable our residents to maintain independence, build confidence, and achieve their rehabilitation goals.
Responsibilities:
Conduct thorough assessments of residents' physical, cognitive, and emotional abilities to identify individualized therapy needs
Develop and implement personalized occupational therapy plans to address identified needs, focusing on activities of daily living
Utilize various therapeutic modalities, such as cognitive, motor, and sensory integration strategies, to promote resident independence and function
Collaborate with the interdisciplinary team to ensure comprehensive care and effective communication
Conduct regularly scheduled therapy sessions, with flexibility to adjust schedules as needed
Maintain accurate and timely records of therapy sessions, including progress notes, treatment plans, and billing information
Participate in facility-wide initiatives and committees to promote resident-centered care and quality improvement
Requirements:
Master's or Doctoral degree in Occupational Therapy from an accredited program
Current licensure or certification as an Occupational Therapist in the state of Illinois
Minimum 1 year of experience as an Occupational Therapist in a long-term care setting, preferably in a nursing home environment
Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals
Ability to lift, bend, and move frequently throughout the workday
