As a Licensed Practical Nurse (LPN), you will:
• Provide oversight, coordination, and intervention for health-related needs of program participants
• Maintain nursing documentation and medication records
• Assist individuals in understanding their medications and ensure proper medication administration and documentation in the electronic MAR system
• Provide direct nursing care including wound care, tracheostomy care, diabetic counseling, and related health services
• Assess individuals' physical health and emotional well-being
• Collaborate with nursing staff, program staff, healthcare providers, and interdisciplinary teams
• Communicate significant health concerns to nursing leadership and program management
• Maintain confidentiality and protect the privacy and dignity of individuals receiving services
• Support the overall health, safety, and wellness of participants throughout the program

• Licensed Practical Nurse (LPN) with a current Connecticut nursing license
• Verification of eligibility to practice in Connecticut
• Current CPR and First Aid certification
• Required FBI and other background clearances
• Completion of all training requirements associated with licensure
• Strong communication and documentation skills
• Experience supporting individuals with intellectual and developmental disabilities
• Experience in community-based healthcare or residential services
• Experience with electronic medication administration and documentation systems
• Full-Time
• Flexible scheduling based on program needs
• Evening, weekend, and holiday availability may be required
• Additional hours may be available based on operational needs
• Ability to exert up to 50 pounds of force occasionally
• Ability to stand, sit, walk, bend, kneel, stoop, crouch, crawl, and climb stairs
• Ability to reach, push, pull, and perform repetitive motions
• Ability to use computers, mobile devices, and agency software systems
• Ability to operate a vehicle and travel between service locations as needed
• Ability to perform essential job functions with or without reasonable accommodation
