About the Licensed Practical Nurse LPN Role:
We are seeking a compassionate and skilled Licensed Practical Nurse LPN to work in our skilled nursing facility. As an LPN at Pine Haven Nursing & Rehabilitation Center, you will be responsible for:
Providing direct patient care to our residents, including administering medications, performing vital signs, and monitoring their health status
Collaborating with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of our residents
Maintaining accurate and up-to-date medical records and reporting any changes in a resident's condition to the healthcare team
Licensed Practical Nurse LPN Responsibilities:
Provide high-quality patient care in accordance with established nursing standards and protocols
Conduct initial and ongoing assessments of residents' physical and emotional needs
Develop and implement individualized care plans with input from the healthcare team
Administer medications and treatments as prescribed by physicians
Maintain a clean and safe environment for our residents
Participate in quality improvement initiatives to enhance the overall quality of care

Licensed Practical Nurse LPN Requirements:
Current licensure as a Licensed Practical Nurse (LPN) in the state of New York
Previous experience in a skilled nursing facility or long-term care setting preferred
