Position Overview

Casper Mountain Rehabilitation & Care Center is seeking a compassionate, dependable, and clinically skilled Licensed Practical Nurse (LPN) to join our skilled nursing team.

As an LPN, you will provide direct nursing care, administer medications and treatments, monitor resident conditions, and support the coordination of individualized care. You will work closely with Registered Nurses, CNAs, physicians, and other members of the interdisciplinary team to ensure residents receive safe, timely, and person-centered care.

Our LPNs are trusted members of the clinical team who provide both hands-on nursing care and frontline leadership. This position is ideal for a nurse who values resident relationships, teamwork, clinical excellence, and the opportunity to make a meaningful difference every shift.

Key Responsibilities

Clinical Care & Resident Oversight

  • Provide direct nursing care in accordance with individualized care plans, physician orders, facility policies, and established clinical standards

  • Monitor residents for changes in physical, cognitive, behavioral, and functional condition and promptly communicate significant findings

  • Administer prescribed medications, treatments, and therapeutic interventions within the LPN scope of practice

  • Monitor resident responses to medications, treatments, and nursing interventions

  • Obtain and evaluate clinical information, including vital signs and other resident observations, as appropriate

  • Notify the appropriate RN, physician, provider, or nursing leadership of significant changes in resident condition

  • Assist with admissions, readmissions, transfers, and discharges as assigned

  • Ensure physician orders, treatments, and nursing interventions are implemented accurately and timely

  • Respond appropriately to urgent resident needs and emergency situations in accordance with established procedures

  • Promote resident dignity, independence, comfort, preferences, and quality of life

Nursing Leadership & Team Support

  • Supervise, delegate, and provide clinical direction to CNAs and other nursing support personnel within applicable scope of practice

  • Monitor completion and quality of assigned resident care

  • Provide guidance, coaching, and support to CNAs and other members of the nursing team

  • Promote accountability, teamwork, professionalism, and respectful communication

  • Assist with maintaining appropriate workflow and resident-care priorities throughout the shift

  • Communicate staffing, resident-care, performance, and safety concerns to nursing leadership

  • Support the orientation and development of new nursing team members as assigned

  • Serve as a clinical resource and positive role model for nursing support staff

Care Coordination & Communication

  • Collaborate with Registered Nurses, physicians, therapy, pharmacy, dietary, social services, and other interdisciplinary team members to coordinate resident care

  • Communicate changes in resident condition and treatment needs to appropriate members of the care team

  • Participate in resident care planning and interdisciplinary meetings as assigned

  • Provide residents and families with appropriate education regarding medications, treatments, and care needs

  • Maintain professional and compassionate communication with residents and responsible parties

  • Ensure effective shift-to-shift communication to promote continuity and consistency of care

  • Follow through on physician orders, laboratory results, diagnostic findings, and clinical recommendations as appropriate to the position

Documentation & Regulatory Compliance

  • Maintain accurate, complete, and timely documentation within the electronic medical record (EMR)

  • Document medications, treatments, resident observations, changes in condition, physician notifications, and nursing interventions as required

  • Maintain compliance with applicable CMS requirements, Wyoming regulations, facility policies, and professional nursing standards

  • Protect resident rights, privacy, dignity, confidentiality, and personal preferences

  • Immediately report accidents, incidents, allegations, safety concerns, and other reportable events according to facility policy

  • Support ongoing survey readiness through accurate documentation and consistent adherence to established clinical practices

Quality & Resident Safety

  • Follow infection prevention and control practices, including hand hygiene, PPE requirements, and applicable transmission-based precautions

  • Participate in Quality Assurance and Performance Improvement (QAPI) activities as assigned

  • Support facility initiatives related to fall prevention, infection prevention, skin integrity, medication safety, and reduction of avoidable hospitalizations

  • Identify potential resident safety risks and take appropriate action within scope of practice

  • Maintain a clean, organized, and safe resident-care environment

  • Promote a culture of clinical accountability, resident safety, and continuous improvement

Qualifications
  • Graduate of an accredited practical nursing program

  • Current, active Licensed Practical Nurse (LPN) license in good standing in the State of Wyoming

  • Current CPR or Basic Life Support (BLS) certification

  • Previous skilled nursing, long-term care, rehabilitation, or post-acute nursing experience preferred

  • New graduate LPNs who meet applicable licensing requirements are encouraged to apply

  • Strong clinical observation, critical-thinking, and decision-making skills

  • Ability to recognize and appropriately respond to changes in resident condition

  • Ability to appropriately supervise, delegate, and support nursing assistants

  • Excellent written and verbal communication skills

  • Strong organizational and time-management abilities

  • Ability to manage multiple clinical priorities in a fast-paced healthcare environment

  • Compassionate and professional approach to caring for older adults and individuals with complex healthcare needs
