Physical Therapist (PT) Lead – 13 Week Contract
Location: Northbrook, IL 60062
Position Type: 13-week contract
Pay Range: $2,159 – $2,421 (weekly)
Experience Required: 1+ year Home Health (HH) experience + HCHB (Homecare Homebase) experience required
Territory Coverage: Chicago team & Lake Forest region
Schedule: 12-hour day structure as needed (home health visits; schedule varies by patient census)
Licensure: Must meet Illinois PT licensure requirements
Position Summary & Responsibilities:
Provide in-home physical therapy evaluations and treatment plans
Manage patient caseload across Chicago and Lake Forest territories
Document visits and care plans using HCHB system
Coordinate care with interdisciplinary home health team
